You will require a blood form from your doctor to take with you to your appointment.

If you are attending for a fasting (starved) blood test it is important that you DO NOT EAT FOR 12 HOURS BEFORE YOUR APPOINTMENT.  A drink of water is permissible on the day.

If you are unsure whether you should fast or not please contact the surgery who will advise you.
