Some services provided are not covered under the NHS contract and therefore incur charges. These include private sick note, insurance forms, holiday cancellation paperwork, medical reports, fitness to travel certificates, application for firearms, HGV and some vaccination services.

Copies of notes for solicitors may be chargeable if deemed large/excessive of if duplicates are required. You can sign up to SystmOnline/NHSapp and view your medical records online.		  

If the service, you require is not listed below please ask at reception and a quotation can be provided.

Please note payment will be required in advance by card or cash.

We are unable to countersign passport and driving license applications.

The work listed above is non NHS work and is often carried out outside of normal working hours and the waiting time is therefore affected.

The above prices include 20% VAT where applicable.
