New Patients

To register with the practice please visit Crich Medical Practice, Oakwell Drive, Crich to complete the registration documentation. Please note we are unable to accept registrations at Holloway and South Wingfield branch surgeries.

NHS number is required – we will be unable to complete your registration without it

Patients who move out of our catchment area after 01/01/2020 will be asked to register at a closer GP Surgery, this applies to all patients without exception.
